Certified Home Health Agency (CHHA)
Home Care Services:
- Enables patients and families to maintain or restore independence at home.
- Accredited by New York State Department of Health as a Certified Home Health Agency
- Services are provided by professionals who specialize in caring for people at home. Registered nurses, home health aides, personal care aides physical therapists, occupational therapists, speech therapists respiratory therapists, medical social workers and dieticians.
Services provided under Certified Home Health Agency (CHAA):
- Wound Care
- I.V. Therapy
- PRI and screens
- Rehabilitative Care
- Disease Specific Care and Instruction
- Case Management
- Medication Management
- Injections
- Venipunctures
- Bath/Shower Assistance
Payment for Services:
We bill Medicare, Medicaid Veterans Insurance, Private Insurance Companies such as Blue Cross, and HMO’s. Private payment is available on a sliding scale basis.

We are available evenings weekends and holidays. To reach us after hours call the Sheriff’s Office at (607) 746-2336 and ask for the on-call nurse.
Long Term Home Health Care Program
(LTHHCP)
What is the Long Term Home Health Care Program?
The LTHHCP is an alternative to nursing home placement and a program to assist people of all ages to remain in their home during a long illness or recuperation.
It is also a coordinated plan of health and personal services tailored to meet a specific person’s health, social and
environmental needs. The LTHHCP assists relatives or friends in caring for persons who qualify for this program.
Who is eligible?
A frail, disabled, or chronically ill person of any age group is eligible if he or she:

Is medically eligible for nursing home care in a skilled nursing facility (SNF) or a health related facility (HRF).
- Has an assessment completed by the LTHHCP team which indicates the client’s needs can be safely met through The Long Term Home Health Care Program.
- Has his or her physician’s approval for the Home Care Program.
- Is a Delaware County resident.
What is the cost?
The cost of the home care should not exceed 75% of the cost for care in a skilled nursing facility or health related facility in Delaware County.
